REQUIRED WAIVER FOR ALL PARTY GUESTS

Please click on the date of the party you are attending and sign a waiver; you only need to do this once; if you already know (confidently) that you have a waiver on file with us, you do not need to do this again.
**Please note, Hi-Five Sports Zone is a private business and ALL children under the age of 18 that enter our facility must have a waiver on file whether they plan play or not. Thank you for your cooperation!
  1. Click on the date of the party or program your child/children are attending.
  2. Click “Add to Cart”.
  3. Add all children, and fill out required information.
  4. Initiate “Checkout”. There is NO payment information required for this.
  5. Your child/children are now covered by the waiver and you’re all set! Just give your child's first name at the front desk when you check in.
